You have a failed Nvidia GPU chip, this series had a lot of problems with this chip. Sadly, its time for a newer system ;-{
I have had some luck with doing a reflow of the chip with a hot air work station. You can get a cheap one such as this, try it then resell it on ebay if you have no further use for it. You might even be able to call a tool rental place. http://www.ebay.com/itm/2-in1-898D-Solde…
